Gauteng Nurse Shortage Crisis: Public Health System Loses Up to 100 Nurses Monthly to Private Sector and Overseas

GAUTENG — The Gauteng public health system is facing a severe nurse shortage crisis, losing up to 100 nurses every month to the private sector and overseas opportunities. According to Bongani Mazibuko, Provincial Secretary of the Democratic Nursing Organization of South Africa (DENOSA) in Gauteng, this mass exodus is placing unsustainable pressure on an already stretched healthcare workforce, outpacing the rate at which training institutions can produce new graduates.

The Gauteng Department of Health’s acting Deputy Director-General, Sabonga, has acknowledged that the public health system is losing nurses faster than they can be replaced, further compounding the strain on frontline staff. However, DENOSA emphasizes that this crisis is driven by systemic failures to recruit, fund, and deploy healthcare workers effectively.

Outdated Salaries and Training Bottlenecks
A primary catalyst for the exodus is the Occupation Specific Dispensation (OSD) for nurses, which was implemented in 2007 and has never been reviewed. Mazibuko stressed that the current salary structure no longer reflects the realities of nurses on the ground, making them prime targets for international recruitment. Foreign entities from Australia, the United Arab Emirates, and Saudi Arabia are actively poaching South African nurses with lucrative contracts, recognizing the high quality of their training.

Compounding the salary disparity is a bottleneck in the nursing training pipeline. The current curriculum produces graduates with a three-year diploma or degree focused primarily on general nursing, or a four-year university degree that includes midwifery. This is a departure from the previous comprehensive four-year curriculum, which included training in general nursing, community nursing, psychiatry, and midwifery. Under the current system, a nurse must first obtain a midwifery qualification before specializing in other nursing spheres, creating an unnecessary delay in producing specialized healthcare workers.

Unmanageable Workloads and Workplace Violence
The shortage translates directly into dangerous working conditions. Nurses in clinics are frequently forced to manage 50 to 60 patients at a time, while professional nurses in hospital wards have been cited as being responsible for 40 to 50 patients simultaneously.

Mazibuko noted that these unmanageable ratios lead to extended patient wait times and mounting frustration. Tragically, this environment often results in patients lashing out, leading to an increase in workplace violence and physical attacks against nurses. These are not isolated incidents, but symptoms of systemic issues that the department has failed to address.

Systemic Mismanagement and Outdated Data
DENOSA asserts that this is not merely a provincial mismanagement issue, but a national policy gap requiring urgent intervention from the Department of Public Service and Administration (DPSA) and the national Department of Health to review the OSD.

Furthermore, the Gauteng Department of Health is currently sitting with over 2,000 nursing vacancies. Mazibuko criticized the department’s slow recruitment processes, questioning why vacant posts are not being advertised and filled immediately. He revealed that the department’s current staffing projections are based on an organogram that was last reviewed in 2006. A thorough, modern audit is required, as the actual shortage is likely far greater than the reported 2,000.

Additionally, DENOSA has called for an immediate cleanup of the PERSAL personnel system. Audits have revealed that positions budgeted and designated for professional nurses are currently being occupied by administrative staff or cleaners, masking the true extent of the nursing deficit.

Threat to National Health Insurance (NHI)
Looking ahead, DENOSA warns that the planned implementation of the National Health Insurance (NHI) is at serious risk. While the organization supports the principles of the NHI, Mazibuko cautioned that the system will fail if the foundational issues of nurse salaries, dilapidated infrastructure, and unsafe working conditions are not resolved first. Nurses are the backbone of the health system, and without them, the NHI cannot succeed.

Despite repeated engagements and public warnings, DENOSA expressed deep disappointment with the department’s recent submissions to the portfolio committee, which offered only vague promises of future planning rather than concrete, immediate action. Mazibuko concluded by demanding consequence management for departmental leadership, stating that a persistent lack of appetite to resolve these known challenges may leave the union with no choice but to take to the streets.
