Retirees Who Move Back to Their Hometown Will Find the Welcome Lasts About a Year

Rural hospital closures top 170 since 2005, and rural areas will meet only 68% of primary care demand by 2037, making cheap housing a false comfort.

A retired couple needs roughly $466,000 to cover their income gap, two years of assisted living, and exit costs if the move doesn't work out.

Building a portfolio and living off one are two completely different skills, and almost nobody teaches the second. This problem is what The Definitive Guide to Retirement Income helps, and it is free today. Read more here. (Sponsor)

Moving back to the town where you grew up feels like the safest retirement move, and that feeling is what makes it risky. Many rural towns are shrinking and aging, rural hospitals have closed, and long-term care remains expensive. Hardly anyone checks out a place they think they already know. So here's what a hometown return actually costs, and why the welcome tends to fade somewhere around the one-year mark.

What follows on the social side comes from experience, and it plays out often enough to plan around. After reunions, people often return to the routines that kept them busy before you showed up. The friends who remain may have decades of birthdays, bowling leagues, and funerals that you missed. Some of the people you came back for may have died or moved away. And shared history can become something you talk about now and then.

The town has changed too. Since 2010, over 1,300 rural counties have lost population, with declines totaling nearly 400,000 people, according to the USDA's Economic Research Service. Those places also are older: 19% of the nonmetro population is 65 or older, compared with 12.0% in metro areas. As a town shrinks and ages, clinics, pharmacies, and home care agencies all struggle to hire.

Many successful investors eventually reach the same moment. The saving is done, the portfolio is built, and the question quietly changes from how much can I grow this to how much can I take out? Get that second question wrong and decades of good investing can come apart in a handful of years.

Healthcare is where nostalgia gets expensive. Since 2005, approximately 180 rural hospitals have closed, according to the North Carolina Rural Health Research Program. By 2037, primary care doctors are expected to cover 73% of demand nationally, but only 68% in rural areas.

Long-term care remains expensive even as housing costs fall. Genworth and CareScout's 2024 survey found national median values of $70,800 a year for assisted living, $77,792 for a home health aide, and $111,325 for a semi-private nursing home room. In a small town, the bigger question is often whether there's a bed or an aide available at all.

Picture a couple at 67 who own their hometown house outright. Medicare Part B, coverage for doctor visits, costs $202.90 per person per month this year. That's $4,869.60 for the two of them before any supplemental plan. Here's a working budget:

Transportation, including drives to specialists and replacing the car

The average retirement benefit is about $2,079 a month, just under $24,950 a year. If both spouses get roughly that, they bring in $49,900 a year, and the 2027 cost-of-living raise is tracking toward 3.3%. That leaves a $12,100 gap. At a 4% withdrawal rate, which matches a traditional 30-year retirement, it takes about $302,500 to cover it. If the higher earner waits until 70 to claim, the benefit grows, and that number gets smaller.

Selling a $600,000 city house and buying a $300,000 one back home feels like a win. It also puts your biggest asset in a market with few buyers. National prices are up 1.9% over the past year, and how long homes sit unsold in small towns varies too much to give one figure. Expect fewer buyers when you need to sell.

Agent commissions average 5.46%, and sellers' other closing costs run 1% to 3% on top of that. Using 2% for those costs, the city sale costs about $44,760. Selling the hometown house later costs another $22,380. That's $67,140 lost to the round trip before movers, and you'd also have to explain to everyone why you're leaving the town you picked.

It does work for a lot of people. Those people tend to have specific relationships waiting for them, like a sibling or a best friend they talk to every week, a working hospital within a reasonable drive, and a clear sense of what's changed. Some retirees move to the region instead of the exact town. You stay close to the people and places you know while living near a larger medical center.

Here's the full target: $302,500 to cover the income gap, $141,600 set aside for two years of assisted living, and $22,380 to pay for leaving. That's about $466,000. The plan assumes drawing 4% a year from a portfolio whose returns stay ahead of inflation, and the case for an income-first alternative to that flat 4% rule is laid out in a free report here. If you have less than that, delaying Social Security or picking a regional hub over the old town can narrow the gap.

Before you list your current house, go back for a full winter. Rent a place there. Drive to the closest emergency room and time the trip. Call the home care agencies and ask how long their waitlists are. Find out how long houses like yours take to sell. Then notice who's still calling you in week six.

It may be the least discussed and most consequential transition in personal finance, and plenty of otherwise careful investors walk into it with no real plan.
